The Program of All Inclusive Care for the Elderly (PACE) provides services at a PACE Center. Participants’ social, medical and physical needs are coordinated by a team. PACE focuses on
preventive care in a person’s home. People who join a PACE program must receive their primary medical care through PACE, and will need to see a PACE doctor.

In addition to medical services, PACE participants also receive important social and supportive services, including:

• Day center services and recreational activities
• Social work services
• Medical and day center transportation
• Personal care, meals and home chores
• Assistance with Activities of Daily Living in the center and in your home
• Support groups and respite for family caregivers

Eligibility:

• Financial eligibility requirements to receive services at no cost (See PACE Southeast Michigan website for all financial eligibility details).
• 55 years of age or older,
• Certified by the State of Michigan to need nursing home care
• Able to live safely in the community at the time of enrollment with the support of PACE Southeast Michigan
• Live in the PACE Southeast Michigan service area (as determined by zip code. See their website for full list of zip codes served)
